All team officials are required to have a Police Vulnerable Sector Check (PVSC) which is renewed every 3 years.

Anyone needing a PVSC can download the form and you also need a signed letter from the hockey office to avoid finger printing charges if applicable. Bring both forms to the Community Resource Centre which is listed below or you can use the Online Application link. In some cases, finger printing at the police station may be required. Once you have received your clear PVSC back from the police, bring the original PVSC and your completed Certification Reimbursement Form with all your receipts to the hockey office. 1 reimbursement cheque will be issued when all required documents and certifications are completed so you can be rostered to your team. 
    Volunteers who do not live in York Region will be required to complete a PVSC with the jurisdiction in which they live.

Download the PVSC Form from the York Region Police website. Bring the completed PVSC Form as well as the  PVSC Volunteer Letter from the WSMHA to the Customer Service location below. To request the PVSC Volunteer letter, complete the online request from our website, and a signed letter will be emailed to you. 


Please note: 
Health cards and Social Insurance Number cards are not accepted. Requests for any level of police record check can be made only by the applicant. Third-party requests will not be accepted.

If fingerprints have been taken in the past or it is discovered that fingerprints are necessary for any reason, the online process will stop and the applicant will be asked to attend the Community Resource Centre to complete the application.
