AAA PERMISSION TO SKATE FORMS (PTS FORMS)

Spring 2024 Evaluations

Step 1
Any player wishing to attend a AAA evaluation is required to be registered in the Hockey Canada Registry for insurance and liability reasons. Please register with the WSMHA using our AAA registration link below. There is no cost to register for AAA Evaluations. If the player is selected for a AAA team, please notify us to cancel the AAA registration and then register with the organization that they will be playing for.

We are only authorized to give PTS forms for the Markham Waxers AAA evaluations.

Step 2

Should the player be returning to the WSMHA to play after attending AAA Evaluations, they will then be required to register again on the Hockey Canada Registry website for our hockey program where the registration fee will be applied. Follow the instructions during the registration process on how to make the registration fee payment and take advantage of the summer payment plan. PTS forms are requested electronically via our website at the link below.
PTS forms are completed daily, but can take up to 48 hours. 
The office is closed on Saturdays and Sundays during the off season (after April 01).
You will receive a confirmation email that your request has been submitted. 
Completed PTS forms will be emailed back to the email address provided on the form.
The U10 - U16 PTS Form will be issued to attend the Markham Waxers AAA tryouts.

The U18 AAA PTS form will be issue in accordance with OHF Regulation K1-K27 starting August 23, 2024

Junior Team Permission to Skate Forms
On April 22, 2024 the OMHA has granted blanket permission for all eligible players registered in the OMHA in the 2023-24 season to try out and attend camps with any Junior team under the jurisdiction of the Ontario Hockey Association (OHA) and Northern Ontario Hockey Association (NOHA) for the 2024-25 season.

The Jr. Permission to Skate (PTS) is no longer required. Please advise any player requesting a Jr. PTS that it is no longer required and that the Jr. clubs have been notified via their respective governing bodies
